New research suggests that a condition often overlooked could help identify individuals at risk of developing dementia long before symptoms emerge. The study highlights frailty—a condition marked by decreased physical resilience and increased disease vulnerability—as linked to earlier dementia onset, even among those with minimal Alzheimer’s-related brain changes.
Published in the Journal of Neurology, Neurosurgery & Psychiatry, the study analyzed data from 1,614 older adults from the Rush Memory and Aging Project in Illinois. These participants were monitored over nearly 24 years. Researchers explored how frailty influenced the age of dementia diagnosis.
Frailty was assessed using a frailty index, a measure that considers various health and functional factors. People with scores of 0.25 or higher were deemed frail. The study found that frail individuals were diagnosed with dementia about two to three years earlier than those who were not.
“The best approach to preventing or reducing frailty is a combination of regular physical activity, particularly strength training, and a diet that ensures adequate protein intake,” stated Dr. David Ward, a co-author and research fellow in aging and geriatric medicine at the University of Queensland.
Dr. Ward also recommended consulting with doctors to review medications regularly and manage chronic conditions more effectively. Although the study did not confirm that reversing frailty can prevent dementia, the results indicate that good physical health might delay the clinical onset of dementia.
The findings were consistent across sexes, education levels, and presence of the APOE ε4 gene, a key genetic risk factor for Alzheimer’s disease. Researchers also studied 906 participants who had brain autopsies post-mortem, examining frailty in relation to brain pathology, including Alzheimer’s markers. Among individuals with low dementia-related brain pathology, frailty correlated with an almost 10 percent earlier dementia diagnosis.
Interestingly, this association was not found in those with moderate or high brain pathology levels. The findings indicate that frailty could influence dementia onset independently of brain pathology, suggesting that a person’s physical health might determine when symptoms appear.
Dr. Ward noted that frailty tends to develop gradually, with subtle early signs such as slower walking speeds, diminished strength, less activity, persistent fatigue, or prolonged recovery from illness. He advised consulting a doctor if changes in physical functioning impact daily routines, as medical assessment can pinpoint treatable frailty contributors.
