Cyclosporiasis is a parasitic infection that occurs when consuming contaminated food or water. Recent reports indicate a significant outbreak across the U.S., impacting thousands, making it the largest ever seen in the country. As of July 28, the Centers for Disease Control and Prevention noted cases in 45 states.
Outbreak Details
Michigan has been heavily affected, with over 10,000 cases, likely linked to contaminated lettuce carrying microscopic Cyclospora parasites. These parasites rely solely on human digestive systems to reproduce. They may have traveled via crops from regions where they are common.
Understanding Cyclospora Parasites
The lifecycle of Cyclospora involves dormant stages on produce surfaces. These stages, called oocysts, are tough to detect with the naked eye. According to Vernon Carruthers from the University of Michigan Medical School, oocysts travel through the digestive tract, where they activate in the gut due to body temperature and digestive enzymes.
Upon activation, they attach to the small intestine, rapidly replicating and causing damage to intestinal cells. Over time, they form new oocysts in a complex reproduction process within the infected cells.
Symptoms and Transmission
The destruction of intestinal cells leads to symptoms like nausea and diarrhea. A single milliliter of diarrhea can contain thousands of oocysts. Transmission occurs through ingestion of these pods found in contaminated food or water. However, freshly excreted pods aren’t immediately infectious; they need time to mature outside the body.
The parasites can contaminate food systems via polluted soil or water, and improper handling. Industrial washing of produce can further spread oocysts. Resistant to chlorine, sunlight, and cold, they remain tenacious in such environments.
Treatment and Immunity
Infections can persist for weeks or months. Unlike short-lived viral infections, Cyclospora requires specific antibiotics to prevent replication and assist the immune system. Although not often fatal, exposure can lead to immunity in regions where the parasite is common. Studies show that young people in endemic areas are more susceptible to symptoms.
Current Challenges
Recent years have seen increasing Cyclospora cases in the U.S. The current spike in 2023 is significant, with major outbreaks noted in previous years, notably 2018 and 2019. The rise is primarily tied to imported food from countries lacking proper sanitation. Cyclospora can enter the food supply through contaminated groundwater or crops.
Yet, why this year has seen such a marked increase remains uncertain. According to Dr. Bobbi Pritt from the Mayo Clinic, this could be due to contaminated products reaching popular restaurants. For example, lettuce from Taylor Farms, linked to this outbreak, was recalled after illnesses were noted in multiple states.
Health officials continue to trace other potential sources, such as parsley and cilantro in North Carolina. While investigations continue, individuals are advised to wash and cook fresh produce to reduce infection risks.
